Access to Policy Connect may be required for a number of reasons including:

• Provide guidance to a person wanting to become a volunteer (most common reason)
• Provide access to Enjoy Church policies

The form should only be completed for a person that has been created on Elvanto. If the person does not exist on Elvanto, please add them before completing this form.

Once the form is completed for a person the access will be provided and an email sent from Ideagen Mazlan Home (app@hub.ideagen.com) containing instuctions for signing in to Ideagen Mazlan Home.
